A 24-week clinical trial led by Ain Shams University researchers found that weekly dulaglutide improved glycemic control more effectively than daily insulin in 80 adolescents aged 10–18 with thalassemia-dependent diabetes unresponsive to metformin.

Researchers have identified a weekly GLP-1 receptor agonist—dulaglutide—as a potentially superior alternative to daily insulin for managing blood glucose in adolescents diagnosed with diabetes linked to transfusion-dependent beta-thalassemia (TDT). The findings emerged from a controlled clinical trial conducted by investigators at Ain Shams University in Egypt.
The study enrolled 80 adolescents aged 10 to 18 years who had developed diabetes as a complication of beta-thalassemia and whose condition had not responded adequately to metformin. Participants were randomly assigned to one of two treatment arms: one received weekly subcutaneous injections of dulaglutide, while the other received daily insulin therapy. The intervention period lasted 24 weeks.
Participants receiving dulaglutide demonstrated greater improvement in glycemic control compared to those on insulin. Specifically, they exhibited reduced glucose variability, lower mean glucose levels, and increased time spent within the target glucose range. Unlike the insulin group, the dulaglutide cohort showed no clinically significant weight gain during the trial period. Preliminary signals also suggested the drug may contribute to decreased iron accumulation in the body—a critical consideration in TDT patients.
GLP-1 medications function by stimulating the pancreas to secrete its remaining endogenous insulin, rather than replacing insulin externally. This physiological approach is thought to underlie the observed advantages in glucose stability and metabolic profile, particularly in patients whose pancreatic function has been compromised by iron overload—a hallmark of chronic transfusion therapy in severe beta-thalassemia.
The research team emphasized that these results remain preliminary. They called for larger, multicenter trials with extended follow-up periods to confirm dulaglutide’s efficacy and safety profile in this population—and to further investigate its potential impact on systemic iron burden. The study was published in the journal Diabetologia.
