A recent study has identified two simple physical measurements—abdominal diameter and relative grip strength—as jointly linked to a near-doubling of all-cause mortality risk. Researchers in China followed the health status of 5,674 adults in the United States for approximately seven years, during which 373 deaths occurred.

What the data showed

The analysis revealed that individuals exhibiting both increased abdominal fat—proxied by abdominal diameter—and relatively reduced grip strength faced a 97% higher risk of death from any cause compared to those without either factor. Abdominal fat was not measured directly; instead, researchers used abdominal diameter as a surrogate indicator. Abdominal diameter alone did not show a statistically significant association with elevated mortality risk. In contrast, weaker grip strength—when adjusted for body size—was associated with a roughly 60% increase in mortality risk.

When both factors co-occurred—elevated abdominal diameter and comparatively low grip strength—the combined effect corresponded to an almost twofold rise in all-cause mortality risk. This association remained statistically significant even after adjusting for age, socioeconomic status, lifestyle factors, and comorbid conditions.

Why relative grip strength matters

Prior research has examined the relationship between handgrip strength and mortality, but most such studies focused on absolute grip strength values. The new study instead normalized grip strength against body size to identify individuals whose muscular strength is disproportionately low relative to their physical dimensions.

Researchers suggest abdominal diameter may more accurately reflect visceral fat accumulation than other anthropometric measures. Meanwhile, relative grip strength may help detect sarcopenia—or age-related muscle loss—in individuals with larger body frames, where absolute strength metrics could mask underlying weakness.

Limitations of BMI and the case for composite assessment

These findings emerge amid growing scrutiny of body mass index (BMI) as a standalone health indicator. BMI does not differentiate between fat distribution or quantify lean muscle mass, potentially obscuring critical health risks. The authors argue that combining multiple simple measurements may yield a more comprehensive clinical picture than BMI alone.

Brian Plaisance, a kinesiology specialist at the University of Rhode Island, noted that BMI remains useful—but emphasized it should be interpreted alongside waist circumference, weight trends, muscular strength, physical function, and overall health status.

Dynapenic obesity and clinical implications

The coexistence of excess adiposity and diminished muscular strength is termed “dynapenic obesity.” Earlier analyses using waist circumference to define this condition reported a 73% increase in mortality risk. However, the researchers stress that further evidence is required before abdominal diameter and relative grip strength can be adopted as routine clinical screening tools. No single measurement, they caution, can definitively predict an individual’s future health trajectory.
