A Korean study finds that replacing traditional cigarettes with high-nicotine e-cigarettes increases the risk of severe eye conditions compared to quitting nicotine entirely.

Researchers have identified a connection between switching from conventional cigarettes to electronic cigarettes, instead of quitting nicotine altogether, and an increased risk of serious eye diseases that may lead to vision loss.
A large-scale Korean study revealed that individuals who replace traditional smoking with high-nicotine e-cigarettes—often marketed as a less harmful alternative—face a higher likelihood of developing eye problems compared to those who completely stop using nicotine.
The eye conditions involved include various diseases such as diabetic retinopathy, which can cause blindness due to damage to the blood vessels in the retina. Other disorders include myopia, hyperopia, and astigmatism, which result in blurred vision. More severe diseases in this group encompass cataracts, glaucoma—which harms the optic nerve—and age-related macular degeneration affecting central vision.
The researchers suggest that nicotine itself may be the primary factor behind these effects. Nicotine causes blood vessel constriction and reduces oxygen flow to the sensitive tissues of the eye, while also triggering chronic inflammation, even without the toxic substances produced by tobacco combustion.
Over a follow-up period of approximately 4.6 years, the study recorded more than 6,300 cases of serious eye diseases. The lowest incidence rates were observed among those who quit nicotine entirely, while rates were higher among e-cigarette users. The data did not include individuals who continued traditional smoking.
Overall, switching to e-cigarettes was associated with a 7% increase in the risk of eye diseases that impair vision compared to complete nicotine cessation.
This risk increase was not uniform across all diseases. Diabetic retinopathy stood out in particular, with a 24% higher risk among e-cigarette users compared to those who stopped nicotine use.
The study utilized data from South Korea's National Health Insurance Service, which covers about 97% of the population. It involved over 179,000 former smokers, from which approximately 32,000 were selected for the study after adjusting for factors such as age, income, general health, and physical activity.
Participants, averaging around 45 years old, were monitored over 4.6 years, during which five main eye conditions were tracked: cataracts, glaucoma, macular degeneration, diabetic retinopathy, and refractive errors including focusing disorders.
Although the findings were consistent across different groups, researchers cautioned that nearly 98% of participants were male, which may limit the generalizability of the results. Additionally, the follow-up duration might not be sufficient to detect slowly developing diseases.
The researchers concluded that even a slight increase in risk could have significant public health implications due to the widespread prevalence of these eye diseases. They emphasized the need for healthcare providers to inform patients that switching to e-cigarettes is not a risk-free alternative compared to complete nicotine cessation.
The study was published in the American Journal of Ophthalmology.
