Deplin is prescribed to address folate deficiency in patients diagnosed with major depressive disorder or schizophrenia. It contains L-methylfolate, a bioactive form of folic acid that supports the synthesis of neurotransmitters involved in mood regulation.
As a medical food, Deplin is often administered in conjunction with antidepressants to enhance treatment results in depression and to assist in managing symptoms of schizophrenia. The medication is available in two dosages: 7.5 mg and 15 mg, typically taken once daily under medical supervision.
Unlike dietary folic acid, which requires conversion to L-methylfolate to cross the blood-brain barrier, Deplin provides L-methylfolate directly, bypassing this activation step. Once in the brain, it participates in chemical processes that produce serotonin, dopamine, and norepinephrine, neurotransmitters essential for mood balance.
Imbalances in these neurotransmitters have been associated with mental health disorders, including depression and schizophrenia, making adequate folate levels critical for neurological function.
Folate deficiency has been linked to both major depressive disorder and schizophrenia. Research indicates a bidirectional relationship between folate levels and depression, where low folate can increase depression risk, and depression can worsen with folate deficiency caused by poor nutrition.
A 2013 study demonstrated that adding L-methylfolate to antidepressant therapy improved depression symptoms and patient satisfaction. Similarly, a 2021 review of six studies confirmed that combining L-methylfolate or folic acid with primary depression treatments enhanced depression scores, response, and remission rates.
Some patients with treatment-resistant depression possess variations in the MTHFR gene, which reduces folic acid conversion to L-methylfolate. These individuals may particularly benefit from L-methylfolate supplementation alongside antidepressants. Importantly, Deplin itself is not an antidepressant but serves as an adjunct to improve antidepressant efficacy.
In schizophrenia, low folate levels contribute to elevated homocysteine, an amino acid linked to neurodegeneration characteristic of the disorder. A 2018 clinical trial involving 55 schizophrenia patients found that those receiving L-methylfolate for 12 weeks showed symptom improvement compared to a placebo group.
Before prescribing Deplin, healthcare providers review patient medical histories, including allergies to folate or other ingredients. Patients should inform their providers about vitamin B12 levels, red blood cell counts, and any prior reactions to foods, dyes, or preservatives.
Methylfolate supplements are generally safe during pregnancy, but patients who are pregnant, planning pregnancy, or breastfeeding should notify their healthcare providers, as dosage adjustments may be necessary.
Deplin should be swallowed whole, with or without food, and not chewed or divided. If a dose is missed, it should be taken promptly unless it is near the time for the next dose, in which case the missed dose should be skipped to prevent overdose.
The medication must be stored at room temperature in a dry place away from sunlight and humidity, such as avoiding bathroom storage. Patients are advised to adhere to prescribed dosing and not to discontinue or share Deplin without medical advice.
Clinical studies report that side effects of L-methylfolate are generally comparable to placebo, though some users experience agitation, anxiety, or gastrointestinal issues. Patients experiencing these or other non-life-threatening adverse effects should consult their healthcare provider for possible dosage adjustments or alternative treatments.
Allergic reactions to Deplin are rare but can include difficulty breathing, dizziness, skin rash, and swelling of the face, tongue, or throat. Immediate medical attention or emergency services should be sought if such symptoms occur.
There is insufficient research on Deplin’s interactions with other medications. Patients should provide their healthcare providers with a complete list of all drugs, vitamins, and supplements they are taking before starting Deplin. Any new medications initiated during Deplin treatment should also be reported to minimize the risk of adverse reactions.
