Reddish Lunula May Signal Underlying Health Conditions, Study Finds
A clinical review published in the American Journal of Medical Sciences links reddish discoloration of the lunula—the pale, crescent-shaped area at the nail base—to cardiovascular, autoimmune, hepatic, dermatological, and hematological conditions.

The human body sometimes reveals underlying health issues through subtle physical changes that may appear insignificant—such as a shift in the color of the lunula, the pale, half-moon–shaped region at the nail’s proximal base, from its typical white hue to red.
What the Clinical Review Found
A review of documented clinical cases, published in the American Journal of Medical Sciences, identified an association between this reddish lunula and several systemic conditions—including cardiovascular and liver diseases, autoimmune disorders, and arthritic conditions.
Researchers from Rutgers Robert Wood Johnson Medical School in New Jersey conducted the review, compiling reported cases from medical literature dating back to the first recorded description of the phenomenon in 1954.
When Should It Raise Concern?
Red discoloration of the lunula does not automatically indicate disease, nor does it appear in every patient with associated conditions. However, the researchers note that its presence—particularly across multiple nails—may warrant clinical evaluation to rule out latent systemic illness.
Medical case reports have linked the finding to cardiovascular and autoimmune–arthritic diseases, as well as certain hepatic, hematologic, and dermatologic disorders. Documented instances include individuals diagnosed with alopecia areata or carbon monoxide poisoning.
In contrast, isolated redness affecting only a single nail is more likely attributable to localized pathology beneath that nail—such as infection, tumor, or hemorrhage—rather than a systemic process.
What Causes the Red Hue?
The precise mechanism remains unknown. One hypothesis proposes dilatation of capillaries located beneath the nail matrix, resulting in increased blood flow and consequent red appearance of the lunula. Clinicians may apply gentle pressure to the lunula during assessment: if the redness blanches to white, it suggests vascular origin rather than pigmentary change.
The authors emphasize that a reddish lunula is not diagnostic of any specific disease. Rather, it functions as a nonspecific clinical sign meriting attention—especially when present in multiple nails or accompanied by other symptoms.
They attribute the under-recognition of this sign, in part, to its relative rarity—and stress that observing it should prompt clinicians to investigate potential underlying systemic or dermatologic disease.
